Get your REGISTRATION PACK HERE. You will need to have completed the registration form and returned it to the surgery prior to you getting an appointment. We will also require some form on photo ID when submitting your registration form.
CARE NAVIGATION STARTS 5th FEBRUARY
The Practice has completed its training for our Reception Staff to become Care Navigators. This means that they are prepared to help direct you to the best service for your specific health needs. Our receptionists are here to help you get it right. The receptionist might be able to suggest more appropriate professionals such as:
- Family Services
- Support Groups
Please don't be offended if they ask you what the problem is when you ask for an appointment.
SHEFFIELD PRESCRIPTION ORDER LINE
As of the 22nd August 2016, we will be operating a new telephone service to order your repeat prescriptions. The Prescription Order Line (POL) will be open between 9am to 3pm, Monday to Friday and you can contact them on 0114 372 3000.
Trained NHS Healthcare staff will take your phone call and make sure you receive the medications that are suitable for you as well as the correct quantity of medication.
This will mean that the ability to leave a voicemail requesting your medication will be ceased.
PLEASE NOTE: As of the 22nd August 2016, we will no longer accept repeat prescriptions ordered on your behalf by pharmacies unless you meet certain criteria.

WHY YOUR DOCTOR MAY NOT PRESCRIBE YOUR USUAL MEDICINES
NHS Sheffield Clinical Commissioning Group (CCG) is responsible for buying health services on behalf of people in Sheffield, including medicines provided on prescription.
We need to make sure we get the best value for NHS resources and that patients get the most effective care and treatment.
In Sheffield, we currently spend over £3 million a year on medicines that are widely available to buy over-the-counter without a prescription and a further £300,000 on expensive branded medicines where a non-branded product would have been available at a much lower cost.
